I’m looking for a web developer to write a small program in PHP, which generates reports and sends them out via email.

The data for the report will be created by a user completing a web form via a series of drop down boxes or radio buttons. Based on the choices will determine the content of the form, for example, the form may ask “What is your favorite color? “ with a drop down box Red, Blue, Green. If red is chosen then information about the color red is added to the form ..

There will be roughly 15 questions in the form with about 3 or 4 possible answers for each question.

The program will need to collect these answers, generate the predefined text for the report, generate the report in PDF and email it out automatically.

It will need to be possible to change the questions and answers over time but I’m happy to amend PHP for this so an admin interface is not required.
Access to an SQL database is available if required.

The report will need to be formatted with basic graphics like a company logo etc.

The successful programmer should have access to Skype Video and be prepared to chat at least once via this method to allow for a full and effective communication of the project.
