Online Fillable Forms/Documentation Solution for Healthcare Practice - Database Programming Freelance Job

This database should be built as an online application. If the database must be placed and managed on my server, it should be with a host that has adequate eqmt to host and maintain/service the database and should integrate with my exisitng website.
I need an online based documentation solution for my healthcare practice. Allows new Client (or Staff for the client calling in) to log onto our website, create a user name and password, proceed to fill out online forms in the convenience of their home/office. These forms would initially consist of the following (can be provided if needed for quote,but are basic 1-2 pg forms)
a. New Client Form-Demographics + Insurance info
b. History/Health Questionnaire-categorized questions regarding various areas of their health)
c. Exam form (Staff use only)
d. Daily SOAP note form (Staff use only)
e. Treatment Plan and Financial agreement form
This phase can be completed after a basic database and documentation solution is completed.
a. I want the health survey/questionnaire to be “scorable” so that the client can see outcome based results. In other words, each category of health questions will be weighted and scored to show whether there is a need for action, and if so, what the solution(s) options are, based on protocols. If the entire survey is filled out, it will score each category and give summary in a separate report (so the client can schedule to come in and go over the results). On our website, I plan to have a section that allows clients/non clients to “Test Themselves” for areas of interest/categories of health (Allergies, Nutrition, Stress, etc). For those that are not looking to fill out our online forms in order to make an appmt, they can still play with the tests to see where they fall and have some viable solutions. Those that fill out the form in order to come in, will submit and get a call for us to set up the appmt (until we have the online scheduler in place..see phase 3)…and will get results and solutions when they come in for consultation/exam/report of findings
b. the other part of it being scorable, would be to have objective outcome assessments/measurements to use to show medical necessity (for insurance companies to pay quicker). So part of the questions within the History/Health Questionnaire would be for “outcome assessment”. Project must have ability to add on in the future. I plan to have additional phases that completely integrate all of the practice needs. The database should be on a suitable platform that would accommodate all aspects of the features that will be added. The goal for the database platform would be to seamlessly integrate all areas of the business/practice including, but not necessarily limited to the main categories:
a. Documentation
b. Billing
c. Accounting
d. Collections
e. Contact Software (emails, fax, IM, TM, rolodex, tasks, goals and project mgr, calendar/scheduler)
f. Marketing
